Stoke proporcionará más resultados sobre STK-001 en la Reunión Anual de la Sociedad Americana de Epilepsia

En la 30ª Conferencia Anual de Sanidad de «Credit Suisse», Stoke Therapeutics ha hablado sobre los datos provisionales que presentaron en septiembre y su presencia en la Reunión Anual de la Sociedad Americana de Epilepsia (AES) 2021, que tendrá lugar del 3 al 7 de diciembre de 2021 en Chicago, Congreso en el cual la Fundación también estará presente organizando una sesión científica.

Durante la Reunión Anual de la AES informarán sobre los datos del STK-001. Como ya os hemos comentado anteriormente, STK-001, un oligonucleótido antisentido patentado, es un nuevo medicamento en investigación para el tratamiento del síndrome de Dravet que se está evaluando en ensayos clínicos. La función de STK-001 es aumentar la expresión de la proteína NaV1.1 mediante la copia no mutante del gen SCN1A ―por lo que daría igual el tipo de mutación en la otra copia― para restaurar los niveles fisiológicos de NaV1.1 y restablecer la conductividad eléctrica. De esta manera pretenden reducir tanto la aparición de convulsiones como las comorbilidades más significativas.

Los datos preclínicos de Stoke Therapeutics demuestran resultados prometedores y los llevarán a la Reunión Anual de la AES este diciembre, donde hablarán de:

El estudio BUTTERFLY

El estudio BUTTERFLY es un estudio de historia natural con 36 pacientes inscritos donde miden las convulsiones, el sueño, la cognición y el comportamiento. Los resultados iniciales de este estudio mostraron:

  • La validación de las medidas cognitivas estándar para su uso en pacientes con síndrome de Dravet.
  • Disminución sustancial de las capacidades neurocognitivas a pesar del uso de múltiples terapias antiepilépticas.
  • Desviación de los niveles normales del desarrollo intelectual general que aumenta con la edad.
  • Una brecha en el funcionamiento adaptativo.

El estudio MONARCH

Este estudio del STK-001 se encuentra en fase 1/2a y se está llevando a cabo en Estados Unidos. Durante la conferencia incluirán datos de la reducción de las convulsiones del STK-001 cuando suministran dosis únicas y múltiples ascendentes de STK-001 hasta 45mg. Las dosis mayores de 45 mg siguen en espera para que sean aprobadas por la FDA. Los datos previos muestran como las convulsiones se reducen en 8 de los 11 pacientes que tomaron una única dosis baja. En el congreso de AES 2021 presentarán más datos sobre los resultados de las dosis múltiples ascendentes desde 20 mg a un total de 21 pacientes. Además, parece que es bien tolerado en los pacientes.

Además, proporcionarán más actualizaciones sobre la seguridad y tolerabilidad de los niveles de dosis únicas y múltiples ascendentes, la farmacocinética humana y la exposición al fármaco en el líquido cefalorraquídeo.

El estudio ADMIRAL

Este estudio se lleva a cabo en el Reino Unido, donde los pacientes reciben múltiples dosis de STK-001, comenzando con 30 mg. Según Stoke Therapeutics, el plan es llegar hasta 70 mg. Durante el Congreso de la AES 2021 compartirán el diseño de este estudio y los datos de seguridad y tolerabilidad de múltiples dosis ascendentes, la farmacocinética humana y la exposición al fármaco en el líquido cefalorraquídeo.

El estudio SWALLOWTAIL

En el estudio SWALLOWTAIL estudian los efectos sobre las comorbilidades no relacionadas con las convulsiones que pueden surgir del tratamiento con STK-001. Los pacientes de este estudio ya habían participado previamente en estudios de STK-001.

En el congreso de la AES 2021, Stoke Therapeutics dará una actualización de la revisión de los pacientes inscritos y los datos de seguridad cuando éstos reciben dosis múltiples. Seguirán a esos pacientes a lo largo del tiempo para poder continuar con las actualizaciones sobre los efectos de este tratamiento.

En la nota de prensa que publicaron el pasado viernes, anunciaron los detalles de las presentaciones de Stoke Therapeutics que harán en la AES 2021, siendo los títulos en español los siguientes:

  • Datos provisionales de seguridad, la farmacocinética y exposición al LCR del estudio MONARCH de fase 1/2a de STK-001, un oligonucleótido en antisentido, en niños y adolescentes con síndrome de Dravet.
  • SWALLOWTAIL: Un Estudio de Extensión Abierto para Pacientes con Síndrome de Dravet que previamente participaron en estudios de STK-001.
  • ADMIRAL: Un estudio abierto en el Reino Unido para investigar la seguridad y la farmacocinética de múltiples dosis ascendentes del oligonucleótido en antisentido STK-001 en niños y adolescentes con síndrome de Dravet.
  • BUTTERFLY, un estudio observacional para investigar la cognición y otras comorbilidades no convulsivas en niños y adolescentes con síndrome de Dravet.
  • Un modelo farmacocinético para STK-001, un oligonucleótido en antisentido, basado en datos de primates no humanos (NHP) permite la selección de la dosis en pacientes con síndrome de Dravet.

Por otro lado, en la 30ª Conferencia Anual de Sanidad hablaron sobre el STK-002, un nuevo candidato clínico para el tratamiento de la atrofia óptica autosómica dominante (ADOA), el trastorno hereditario más común del nervio óptico. Esta grave y progresiva enfermedad actualmente no tiene tratamientos aprobados y, al igual que el síndrome de Dravet, la ADOA está causada por una producción insuficiente de proteínas. El STK-002, un oligonucleótido antisentido patentado, está diseñado para aumentar la expresión de la proteína OPA1 aprovechando la copia no mutante del gen OPA1.

Stoke Therapeutics está en camino de iniciar la recopilación de datos de la historia natural de las personas que viven con ADOA a finales de año. Los datos se utilizarán para comprender mejor la progresión natural de esta enfermedad y así apoyar el futuro desarrollo clínico de STK-002 para el tratamiento de la ADOA.

En la Conferencia virtual sobre salud de Stifel 2021 que hubo el pasado 15 de noviembre, Edward Kaye de Stoke Therapeutics nos habló de la tecnología TANGO (Aumento selectivo de la producción de genes nucleares, del inglés «Targeted Augmentation of Nuclear Gene Output») que utilizan y es propiedad de Stoke Therapeutics.

La aplicación inicial de TANGO es para las enfermedades en las que una copia de un gen funciona de manera normal y la otra está mutada, también llamadas haploinsuficiencias. En enfermedades con haploinsuficiencia, el gen mutado no produce la proteína que debería, lo que da lugar a la enfermedad.

Gracias al método TANGO y a un profundo conocimiento de la ciencia del ARN, los investigadores de Stoke Therapeutics diseñan oligonucleótidos antisentido (ASO) que se unen al ARNm y ayudan a los genes funcionales a producir más proteínas. El objetivo de TANGO es restaurar las proteínas que faltan aumentando ―o activando― la producción de proteínas de los genes sanos, compensando así la copia mutante del gen.

Pero ¿podría un aumento excesivo de la proteína diana dar lugar a efectos adversos? En los modelos preclínicos de Dravet y controles no han observado ninguna evidencia de efectos adversos respecto a convulsiones causados al aumentar considerablemente la expresión de la subunidad Nav1.1. Como el tratamiento se basa en ayudar a los genes funcionales a producir más proteínas, hay un límite de cuánto se pueden sobre-expresar. Además, las subunidades alfa tienen que unirse a la beta para poder generar el canal de sodio, y comentan que seguramente hay una regulación interna que mantiene el límite de canales de sodio que se forman.

The Dravet Syndrome Conference 2023 is organised by Dravet Syndrome Foundation Spain, whose registered office is located at Calle Doctor Fleming, 30, 28036 Madrid, Spain.

The conference shall take place at the Official College of Nursing of Madrid, at Avenida de Menéndez Pelayo, 93, 28007 Madrid, Spain, on Thursday 23rd of March 2023. Attendance to this conference is free of charge, but mandatory for organisational reasons.

Participants must register online at www.dravetconference.com. Upon registration, each participant will receive an electronic confirmation by email. On-site registration will not be be possible. If a participant has not received the official email registration confirmation within 3 days, they must contact the conference secretariat at [email protected].

The conference registration includes: Attendance to all sessions, oral and visual presentations, all coffee and lunch breaks, and conference documentation with all information about the event. The registration does not include travel and accommodation costs or any other personal expenses.

Admittance to the conference without registration cannot be granted.

Conference Attendance
Upon arrival at the Official College of Nursing of Madrid the day of the event, all participants have to check-in at the onsite Conference Registration Desk. The desk will open on 23rd of March at 8:30 a.m. Upon registration onsite, the conference team will provide participants with information about the event and a badge, which should be worn visibly at all times. The personalised badge authorises participants to attend all conference sessions, and lunch and coffee breaks.

Cancellation Policy
Dravet Syndrome Foundation Spain reserves the right to cancel any registration forthwith and without liability on its part in the event of any damage to, or destruction of the venue by fire, shortage of labour, strikes, industrial unrest, public health measures, or any other cause beyond the control of the Dravet Syndrome Foundation Spain, which shall prevent it from performing its obligations. In these circumstances, every effort will be made to find an alternative format/venue or propose a new date for the conference. Should the conference be cancelled for any reason, airfares, hotels, or other costs incurred by the participants are not reimbursed by the Dravet Syndrome Foundation Spain.

Any participant is allowed to cancel his/her registration but must notify this to the conference organisers by e-mail at [email protected] within a minimum of 10 days prior the event. Failure to attend the event without prior written notification by email may incur a no-show fee, which shall cover individual organizational, material and catering costs.

Name Change
All registrations are only valid for the respective person mentioned in the form. A registered participant unable to attend the conference may nominate (free of charge) a substitute participant by notifying the conference organisers by email at [email protected] before 16th of March 2023. The procedure allows giving the last name, first name, and email address of the new participant.

Personal Data
Upon registration, the participant authorises Dravet Syndrome Foundation Spain to use their personal data stated above to inform the participant of any further conferences, events, or information concerning the conference and Dravet Syndrome Foundation Spain’s activities. The conference booklet online will only contain authors, titles, and affiliations of submitted abstracts, but no full abstract texts. The conference printed booklet will contain full abstract texts from all participants. Conference certificates will be sent to the participants as a PDF document upon request at [email protected]. Only the name & affiliation will be stated in the certificate, not the email addresses of the participants.

Information is collected by an automated computer system that manages registrations. Security and confidentiality are guaranteed by state-of-the-art technical tools. However, Dravet Syndrome Foundation Spain declines any responsibility concerning data security transmitted through the internet.

Photography

The conference is held in public; therefore, we do not ban participants, exhibitors, partners and other companies from taking photographs of conference activities and participants, and share it via social media. By participating in the conference, you approve that general pictures will be taken and used for our Social Media output, our websites and other communication purposes. You hereby waive any right to oppose, and expressly authorise the organiser to make them available on the Internet or as it may seem fit. Author’s presentation slides shall not be photographed by any conference participant.

The conference organiser cannot be held responsible for any loss, injury or damage to any property, whatever the cause may be. Should, for any reason outside the organiser’s control (e.g., political or economic circumstances or a case of ‘force majeure’), the venue or speakers change, or the event be cancelled, the organiser will endeavour to reschedule, but shall not be held responsible for any costs, damages or expenses incurred by the participants. If for any reason the organiser decides to make material changes to this conference, the organiser is not responsible for airfares, hotels or other costs incurred by the participants. The participant takes part in the conference, all tours, and trips at his own risk.
