¿Cómo afectó el confinamiento durante la primera ola de la COVID-19 a la comunidad Dravet?

El pasado abril, cuando todos estábamos confinados en nuestras casas para evitar los tan temibles contagios por el coronavirus SARS-CoV-2, los cuales por desgracia siguen ocurriendo, os invitamos a responder a una encuesta desarrollada por varias organizaciones europeas de apoyo a pacientes Dravet, entre las que se incluía nuestra Fundación Síndrome de Dravet.

En dicha encuesta, la cual contó con el aval de expertos médicos de varios países europeos, os hicimos algunas preguntas con la intención de conocer si el aislamiento provocado por el confinamiento que comenzó en marzo de 2020 tuvo algún efecto sobre los pacientes con síndrome de Dravet y también sobre sus cuidadores (vosotros). Las preguntas estuvieron relacionadas con las medidas de protección utilizadas, las crisis epilépticas sufridas por los pacientes Dravet, su comportamiento, la continuación o no de las actividades educativas y de rehabilitación de las personas con Dravet, el acceso o no a la telemedicina, etc.

Fuisteis muchos los que contestasteis a la encuesta y, gracias a los datos aportados por todos vosotros y por nuestras familias amigas en Europa (¡219 familias en total!), hemos podido conocer cuál fue el efecto que tuvo el confinamiento durante la primera ola de la COVID-19 en las personas con síndrome de Dravet (de edades comprendidas entre los 4 meses y los 49 años) y en sus cuidadores. Los resultados son extremadamente interesantes, y os los contamos a continuación:

🛡️ Casi la mitad de familias (48%) ya utilizaban antes del confinamiento ciertas medidas de protección contra infecciones similares (“aislamiento” del paciente” e higiene de los habitáculos) a las recomendadas para prevenir la COVID-19. Estas medidas pre-COVID eran aplicadas por aquellas familias en las que el paciente con síndrome de Dravet tenía crisis febriles frecuentes, posiblemente por el miedo a una posible infección provocara fiebre en el paciente y, como consecuencia, una crisis epiléptica. Sin embargo, no parece haber relación entre el uso de medidas de protección y la frecuencia en sí de las crisis epilépticas. 

😷 La nueva medida de protección adoptada por las familias fue el uso de mascarillas, como recomendaban las autoridades sanitarias. Aun así, su uso por parte de los pacientes Dravet es complicado, por lo que en la mayoría de casos fueron únicamente los familiares/cuidadores convivientes los que se pusieron las mascarillas para proteger al paciente. 

⚡La mayoría de pacientes Dravet (79%) no sufrió cambios en sus crisis epilépticas durante el confinamiento, y sólo algunos (4%) tuvieron que acudir durante este periodo de tiempo al servicio de urgencias, el cual ofreció una asistencia similar a la ofrecida antes de la pandemia.
🤒 Por suerte, solo uno de los 219 pacientes había pasado la COVID-19 en el momento de responder a la encuesta. Este paciente presentó fiebre, pero no aumentaron sus crisis epilépticas debido a la infección.

😠 Sin embargo, el comportamiento empeoró en más de un tercio de los pacientes, aunque este empeoramiento no parece estar relacionado ni con la edad del paciente ni con su frecuencia de crisis epilépticas.

🩺 De los 107 pacientes con consultas que habían sido programadas antes del confinamiento, solo el 11% consiguió dichas consultas; en el 36% de los casos las consultas se llevaron a cabo de manera remota (por teléfono o videollamada), mientras que algo más de la mitad de las consultas terminaron por posponerse. Las consultas telemáticas recibieron evaluación positiva por parte de los encuestados, y los que no pudieron conseguir consulta (ni presencial ni telemática) afirmó que les habría gustado recibir consulta en remoto. Más de la mitad de los encuestados apoya el uso de las consultas telemáticas 📱 incluso una vez que finalice la pandemia. El 55% de los cuidadores opinaron que la mejor manera sería la consulta con vídeo debido a tratarse de un método rápido y efectivo de comunicación. Además, las consultas por vídeo supondrían ventajas tales como la reducción del tiempo de traslado hasta la consulta y los tiempos de espera en las salas comunes de los centros sanitarios.

👐 Sin embargo, no recibieron la misma evaluación positiva las consultas psicológicas remotas, ya que solo 21 familias (9,6%) las recibieron, y éstas fueron por videollamada únicamente en 3 casos (14 casos por llamada telefónica y 4 vía email o SMS). La mitad de las familias que no recibió ayuda en remoto la habría agradecido.

👩‍🏫 El 53% de los pacientes participó en videoconferencias didácticas, organizadas en un 90% por los centros educativos/de día a los que normalmente acuden los pacientes (el otro 10% tuvieron que ser organizadas por las familias).

⚕️ Afortunadamente, ninguno de los ensayos clínicos en los que participaban los pacientes con síndrome de Dravet (20%, siendo 15 de las familias españolas) se interrumpió durante el confinamiento en Europa: los tratamientos se entregaron en las casas o en instituciones accesibles para las familias, y el 64% de las visitas programadas durante dichos ensayos simplemente se pospusieron.

¿Qué conclusiones sacamos de todos estos datos?

✔️ Las familias Dravet están acostumbradas a utilizar ciertas medidas de protección para evitar las infecciones en los pacientes Dravet. Es importante que recordemos que, debido al beneficio conocido que tiene sobre la epilepsia y el comportamiento de los pacientes acudir al colegio y a actividades de trabajo y rehabilitación, y puesto que no se observó una menor frecuencia de las crisis epilépticas cuando los pacientes estuvieron permanentemente en casa, se recomienda mantener en la medida de lo posible dichas actividades en lugar de sobreproteger y aislar a las personas con Dravet.

✔️ El peor comportamiento observado en un tercio de los pacientes durante el confinamiento podría deberse a la falta de actividades y programas de rehabilitación que las personas con Dravet están acostumbradas a realizar, además de al aislamiento en sí que conllevaba un confinamiento. 

✔️ Deben establecerse protocolos de telemedicina para la asistencia a pacientes en situaciones de emergencia, aunque sería también recomendable hacer uso de la telemedicina cuando la familia vive lejos del centro médico y la presencia del paciente en la consulta no sea estrictamente necesaria. De hecho, las citas telemáticas se están ya implementando en los ensayos clínicos.

✔️ Reclamamos también el apoyo psicológico a los cuidadores de personas con Dravet. Dicho apoyo debe existir siempre, pero sin duda alguna se convierte en esencial cuando los cuidadores se encuentran “aislados” con una persona que requiere tantísimos cuidados de su parte. ¡Reclamamos que se cuide al cuidador!

TERMS & CONDITIONS

The Dravet Syndrome Conference 2023 is organised by Dravet Syndrome Foundation Spain, whose registered office is located at Calle Doctor Fleming, 30, 28036 Madrid, Spain.

The conference shall take place at the Official College of Nursing of Madrid, at Avenida de Menéndez Pelayo, 93, 28007 Madrid, Spain, on Thursday 23rd of March 2023. Attendance to this conference is free of charge, but mandatory for organisational reasons.

Participants must register online at www.dravetconference.com. Upon registration, each participant will receive an electronic confirmation by email. On-site registration will not be be possible. If a participant has not received the official email registration confirmation within 3 days, they must contact the conference secretariat at [email protected].

The conference registration includes: Attendance to all sessions, oral and visual presentations, all coffee and lunch breaks, and conference documentation with all information about the event. The registration does not include travel and accommodation costs or any other personal expenses.

Admittance to the conference without registration cannot be granted.

Conference Attendance
Upon arrival at the Official College of Nursing of Madrid the day of the event, all participants have to check-in at the onsite Conference Registration Desk. The desk will open on 23rd of March at 8:30 a.m. Upon registration onsite, the conference team will provide participants with information about the event and a badge, which should be worn visibly at all times. The personalised badge authorises participants to attend all conference sessions, and lunch and coffee breaks.

Cancellation Policy
Dravet Syndrome Foundation Spain reserves the right to cancel any registration forthwith and without liability on its part in the event of any damage to, or destruction of the venue by fire, shortage of labour, strikes, industrial unrest, public health measures, or any other cause beyond the control of the Dravet Syndrome Foundation Spain, which shall prevent it from performing its obligations. In these circumstances, every effort will be made to find an alternative format/venue or propose a new date for the conference. Should the conference be cancelled for any reason, airfares, hotels, or other costs incurred by the participants are not reimbursed by the Dravet Syndrome Foundation Spain.

Any participant is allowed to cancel his/her registration but must notify this to the conference organisers by e-mail at [email protected] within a minimum of 10 days prior the event. Failure to attend the event without prior written notification by email may incur a no-show fee, which shall cover individual organizational, material and catering costs.

Name Change
All registrations are only valid for the respective person mentioned in the form. A registered participant unable to attend the conference may nominate (free of charge) a substitute participant by notifying the conference organisers by email at [email protected] before 16th of March 2023. The procedure allows giving the last name, first name, and email address of the new participant.

Personal Data
Upon registration, the participant authorises Dravet Syndrome Foundation Spain to use their personal data stated above to inform the participant of any further conferences, events, or information concerning the conference and Dravet Syndrome Foundation Spain’s activities. The conference booklet online will only contain authors, titles, and affiliations of submitted abstracts, but no full abstract texts. The conference printed booklet will contain full abstract texts from all participants. Conference certificates will be sent to the participants as a PDF document upon request at [email protected]. Only the name & affiliation will be stated in the certificate, not the email addresses of the participants.

Information is collected by an automated computer system that manages registrations. Security and confidentiality are guaranteed by state-of-the-art technical tools. However, Dravet Syndrome Foundation Spain declines any responsibility concerning data security transmitted through the internet.

Photography
The conference is held in public; therefore, we do not ban participants, exhibitors, partners and other companies from taking photographs of conference activities and participants, and share it via social media. By participating in the conference, you approve that general pictures will be taken and used for our Social Media output, our websites and other communication purposes. You hereby waive any right to oppose, and expressly authorise the organiser to make them available on the Internet or as it may seem fit. Author’s presentation slides shall not be photographed by any conference participant.
